Southview Heights is a proud provider of PACE and Medicaid Waiver Assisted Living Services. These programs offer Medicaid- and PACE-eligible individuals the opportunity to receive financial assistance for the supportive services offered in our home-like environment.
Medicaid Waiver is administered by the Nebraska Department of Health and Human Services. Local Service Coordinators from the Eastern Nebraska Office of Aging or the League of Human Dignity determine eligibility for Medicaid Waiver and work closely with Southview Heights and the Resident to set up a plan of services. The program’s Plan of Services and Supports in conjunction with Southview Heights’ Resident Service Agreement are designed to keep the Resident as independent as possible. The PACE program is run by Immanuel Pathways in Nebraska, and is funded by Medicare and Medicaid. For more information on services and eligibility, please see their website.
